Please help me- aggressive acne and acne scar problems?

Hi guys,

i am Bargavi from TN.before asking your suggestions let me present my case history.please tolerate friends it going to be a lengthy one.

it all started when i was diagnosed with papillary cancer-thyroid at the age of 18 when i was doing college.as per my doctors suggestion i got my entire thyroid gland removed and ever since i am consuming eltroxin-300mg to suppress TSH, calcium-500mg, iron everyday.every  year i stop eltroxin for 30 days, let my TSH rise above 30 and take in a radio-active iodine capsule followed by a whole-body scan after 4 days to make sure my cancer hasn’t come back.after this process i increase my eltroxin intake gradually by taking 100mg-week 1, 200mg-week 2 and 300mg from week 3.

today i am 21. this year’s procedure ended on february 4th.my tsh rose to 93.i am suffering severe hair fall ever since.i ve taken biomin tablets for 3 weeks and now i am taking essvit also using kera xl hair serum for about 10 days but nothing seems to improve.

i am getting pimples which re red, big, takes weeks to disappear  since december and it disappears leaving a big dark spot.as for my regimen i use

aha glow face wash-in morning and night

himalaya neem face wash after getting home

suncote sunscreen and on top of it lakme perfect radince compact(i was using lakme 9 to 5 till january)

my doc prescribed me tretin-0.025% pimple cream which i use only at night.

i don’t use moisturiser because of my oily skin but when needed i use Johnson baby oil.for spots i ve fed up on medical products and currently  i am using only turmeric when i am home.on working days i use spadigaram stone (the one  our grandpa uses after shaving).

i ve tried glyco-6 cream it gave me pimples so did suncros sunscreen and kojivit.benzac was good but it didnt work on my aggressive pimples and on my mild pimples it left a dark dark spot.

please note friends i ve a mild case of dandruff for which i am using biotique-margosa shampoo and i got stress issues too.cos i am a CMA-finalist appearing on june 2014.

any advice ll be useful to me.thank you in advance.

Hi Bargavi, kudos to you girl. You’ve fought through so much struggle, at such a young age too. Acne can come on from iodine as well as Thyroid medication. Iodine has been shown by research to cause acne majorly. The doses you must be getting seem strong enough to do that. Understandably, the treatments cannot be stopped, but you can do things skincare wise that can help. I have a few suggestions for you:

1. Stop using Johnson’s Baby Oil for moisturizing any dry spots on the face. It can be used as a makeup remover, but even then it needs to be washed off thoroughly or it will cause acne.

2. Experiment with using Tretin all over the acne prone areas as a thin layer every night, instead of as a spot treatment.  Given that your doc prescribed it, using it should be fine.

3. Nizoral shampoo twice a week is known to help with dandruff, although if your present shampoo is working for you, that’s okay too.

4. Avoid using any home remedies such as Turmeric or spadigaram for a few weeks and only stick to AHAGlow + Neem face wash + Tretin + Suncote and see if things improve. Sometimes, even without our knowing helpful stuff can cause irritation or clog pores and worsen the acne problem. You could add these things back once you figure out the exact set up that clears your skin.

Keep us posted on how it goes. Figuring out the exact skin care routine takes a bit of trial and error even if one is using acne-safe products only. Good luck!

Ok.thank you so much AM.after i joined this community i do ve understood that the key to a good regimen is to keep as simpler and minimal products as possible.

my dandruff is much in control now and i do hope that my pimples ll be too once my hormones get steady.i know i ve to be patient for dark scars to disappear.

but it is my hairfall that doesn’t let me sleep peacefully at night.every morning i wake up and run my fingers through my hair and atleast 20 strands come along with it and i am dead scared to keep comb on it AM and sometimes even to touch it.

i followed your advice and i stopped using Johnson baby oil.i am currently using cetaphil dam daily advance ultra hydrating lotion.but after looking at the review section i am thinking about using caladril.i remember using it when i was a kid.i simply loved the pinkish tone it gave.

Hi Bargavi, Cetaphil DAM is comedogenic, so please just check all the products you are using with the Skingredients tool. The link is to the top right. For hairfall, I do not have much better suggestions than those already discussed, but checking your vitamin D and other vitamin status might be worthwhile. Vitamin and mineral deficiencies may be caused by strong treatments. Addressing them can get hairfall and other skin issues under control much faster and reliably than topical products alone. Perhaps meet a general physician (family doctor) and ask them for a battery of such tests. You can come to the issue of acne marks and scars once the acne is under control.

Ok thankyou AM. my dermatologist suggested to test my hair follicle if no improvement occurs after a month of biotin-5mg and kera-xl.i might as well ask her this too.

i am applying a thin layer of tretin everyday like you said  i see my dark spots (flat brown hyper-pigmented spots) ve faded slightly and in the past three days i found no new break outs.thank you again AM.will keep you posted  @acnemantra.

Hi @acnemantra . just now i came home after spending six grand for my hair and face.i consulted a different derma today AM.he is the most experienced in my city.

i got my hair root tested.it came positive with dandruff and fungus.he said he’s going to treat me stage by stage.so for the first stage(first month) he has given me Renocia shampoo and solution and a revatilizing hair oil prepared by him.he has given me 3 types of pills,lymecycline capsules hyervita(it’s ingredients all seemed like botanical name) ,tricutis(this one is definitely nutritional supplement).

as for face he has given 4 simple products-simple cos they got only 4 components .clindamycin, benzoeyl peroxide, nicotinamide, and salicylic acid.it seems pretty harsh to me.but it’s just for one month and i do trust him.he cured me when i was little when i got a drug allergy due to nimuloid.

Thank you @acnemantra @catonthewall .i ve been prescribed

morning-vulros soap(bp-2%) and A-CN cream(clindamycin 1%, nicotinamide-4%)

evening and night-saslic DS face wash(salicylic-2%) and night cream-clindoxyl(clindamycin-1%,anhydrous bp-5%).i know i just started yesterday but in contrary to my expectation my skin is getting much oily. let me observe for a couple days and i ll keep you posted friends.

Hey Bargavi, not sure about the other products. But A-CN works but at a decent speed. I am using it only. It heals small pimples in two-three days and big pimples in 5days to a week. But I use A-CN all through the day except when at office.

Thanks for the info @catonthewall .right now i am taking most days off from class to prepare for examinations so i am leaving it on my face for 4-5 hours and then wash off with Saslic DS.A-CN’s base is made of aloe and allantoin makes my skin oily and shiny.

Thank you @dennisdips :-).after a week on my regimen, i still got a couple breakouts but the older&new  ones are healing steadily.the areas around my nose below the eyes use to be extremely oily or extremely dry and the pores would be so open and small white things sticking out of them.

but now the pores seem much tightened and normal,smooth like other areas of my face.i expected my skin to get dry due to bp but it was super duper oily and since yesterday it seems normal.hope the improvement continues.as for hair, the improvement seems pretty on and off.a friend of mine-patient to the same doctor for hair dandruff and fungus said she saw steady improvement during her second sitting.well, i am only on 2nd week of first sitting.so for now, fingers crossed 🙂

That’s so nice to hear that things are working out for you..  I have open pores issue too below my eye area.. My combinations skin isnt divided in the standard T zone and C zone .. Instead my upper half ( imagine a line across my face from one ear lobe to another ) is oily and the area below that which includes chin , jawline and outer cheeks are normal to dry..

I am on Isotretinoin and along with using nadoxin gel as topical antibiotic … Its just been two weeks now and the results are slow.. But  pimples are still popping up..  These are small pimples which then turn into pus filled ones .. I have never had acne issue all my life and then suddenly two months back small acne started popping up and since then i am having a roller coaster ride with acne and dermatologists both .. Starting off with Isotret has been anything but easy.. I spent sleepless nights researching about it , posted questions here and even put forth my concern in front of my dermat ..  I even went for another opinion but his was totally another approach.. So then i had to finally take the plunge..  I will be off it the very first chance i get…

 

By the way what are the products you are using as of now ?

i ve been prescribed

morning-vulros soap(bp-2%) and A-CN cream(clindamycin 1%, nicotinamide-4%)evening and night-saslic DS face wash(salicylic-2%) and night cream-clindoxyl(clindamycin-1%,anhydrous bp-5%).

my previous derma prescribed me isotretinoin too.that’s when i dumped her and went to my current one.he’s the most experienced in my city and he prescribed only lymecycline and the above topicals for my pimples.lymecycline is an anti-bacterial medicine to treat moderate pimples.usual dosage is for 8 weeks.but he prescribed me only for 15 days.i am still getting a couple breakouts now and then too.i am about to tell him during my next visit.but i am not having any second thoughts cos i do see a difference and none of my medicines are anti-biotic.they re all only anti-bacterial.

i you re having second thoughts about something, then better don’t.go with your gut feeling with the isotretinoin Daksh.make your own chance to get off.good luck!

Hi Bargavi, Good Afternoon !

Thanks for sharing your product list.. I used Saslic 1 % and Ahaglow.. Both gave me irritation acne and made my skin reddish.. I told  you na my skin isnt oily as such..Its sensitive combination skin…  I even tried epiduo but BP or may be adapalene dried my skin out giving me red rashes..

A-CN cream sounds interesting.. As it is topical antiobiotic with nicotinamide which helps in acne and marks both.. Can you please tell me the name of the pharma company and its price ? Do you apply it as spot treatment or all over the face ??

Lymecycline as far as I know is tetracycline and tetaracycline / minocycline /doxycycline are all broad spectrum antibiotics and not anti bacterial.. Wait on second thoughts is there any difference between anti bacterial and anti biotic ???

Also you have been lucky to find a good and sensible one.. i consulted three dermats.. first one just gave me azithromycin with nadibact.. but nothing for comedones.. then i visited a very renowned doc in my city.. he prescribed me isotret.. i even went to a third one to have another opinion about this prescription but he just prescribed me vit a tablet for 10 days and dermadew calamine lotion…
its after that i finally decided to go ahead with isotret.. i had no other choice left as on that time.. but i m still in search for a dermat who listens me out patiently and understand my inhibitions..
